Vanderbilt Children's NICU Bridge to Home
Your child may qualify for our Bridge to Home monitoring program. One of our team members will discuss this with you and enroll your child in the program before you leave the NICU to go home. This program allows us to monitor your child’s progress at home using our remote monitoring platform and telehealth appointments.
How We Manage Telehealth Appointments
Our team monitors your child through our telehealth platform partner, Locus Health. At discharge, you will receive an iPad and Bluetooth scale to use while in our remote monitoring program. This iPad will be preloaded with the Locus Health application and the My Health at Vanderbilt application. You will use Locus Health to upload daily information about your child. My Health at Vanderbilt is where your telehealth appointments will take place.
You may also download both applications from the app store to your personal device.
