Requesting Proxy Access

It is important for parents and legal representatives to have access to their child's/dependent's health information. We can help you set up this access. Only parents and legal guardians** can have access.

You must have your own My Health at Vanderbilt account before accessing a child's/dependent's account. If you are currently a patient, you can create your account at myhealthatvanderbilt.com. If you are not currently a patient, you will be sent a sign-up link once we associate your child/dependent to your account.

  • If you are a parent of a child under 13, you do not need to complete an application. You can be granted access to your child's account over the phone by calling 615-936-KIDS (5437) 
  • If you are the parent of a child between the ages of 13-17 or you are a representative other than the parent no matter the child’s age, you must complete an application for each child you are requesting access for.
  • Children aged 13-17 must be present to enroll.

Ask staff for an application, or download a printable version of the age-appropriate form below, complete it, and bring it with you to one of the following locations: 

Once the application has been processed by the My Health at Vanderbilt Team, you can then access your child’s/dependent’s account by logging into your account and clicking on the patient’s name.

**For representatives other than parents, please provide legal documentation to the clinic or fax to (615) 875-2820. This includes: Custody, Health Care Power of Attorney, Conservatorship, Guardianship, Department of Children Services Letter for Foster Parents, etc. You may also submit documentation via secure email to MHAVLegal@vumc.org. Please note that email that is not sent securely (i.e., unencrypted email) may be intercepted and seen by others during transmission. By choosing to send information by unencrypted email, you accept these risks.

Children under 13

If you are a representative other than the parent and requesting access, download, print, and complete application below. Bring the following items to your child's/dependent’s next appointment or location noted above:

  • Completed application form
  • Your government issued photo ID 
  • Legal documentation (see above)

Upon turning 13, access will be revoked until a teen application is completed.

Children age 13-17

If your child/dependent is between the ages of 13 and 17, download, print and complete the application below. Bring the following items to your child's/dependent’s next appointment or location noted above:

  • Completed application form (Ensure your child/dependent signs the application as well) 
  • Your government issued photo ID 
  • If not the parent, legal documentation (see above)

Your child must be present to enroll.

Once the application has been processed by the My Health at Vanderbilt Team, you can then access your child’s/dependent’s account by logging into your account and clicking on the patient’s name. 

Upon turning 18, you will lose access to the child’s MHAV account.

Adults with Diminished Capacity

If your child/dependent is 18 or older and has diminished capacity, download, print and complete the application below. Bring the following items to your child's/dependent’s next appointment or location noted above:

  • Completed application form
  • Your government issued photo ID 
  • Legal documents stating you have conservatorship or health care power of attorney. 

Access to My Health at Vanderbilt will be granted once our Office of Legal Affairs reviews the completed application and legal documents provided. The review can take several business days. We will contact you if we need further information.
